QUARTERLY RECORD SHEET
To help maintain your records with CALM we have provided a link to download and print the Pet Herpetofauna Quarterly Keeper's Record sheet.
This form should be completed every 3 months from the date of your licence issue.
Eg: If your licence was issued on 2 June 2008, your first return should be sent to CALM on 2 September 2008 and every 3 months after that.
Important: If you have received your licence but haven't yet purchased a reptile, this form still needs to be completed with a nil return - ie NIL reptiles on hand.
It is a good idea to mark it on your calendar every 3 months so you don't forget!
